چکیده

Introduction Brugada syndrome (BrS), an inherited ion channelopathy with an autosomal mode of inheritance, is associated with an increased risk of ventricular fibrillation (VF) and sudden death in the young. Implantable cardioverter-defibrillator (ICD) is effective in terminating VF and preventing sudden cardiac arrest in high-risk BrS patients but does not prevent recurrences of VF. An electrical storm occurs in 5% of asymptomatic BrS patients and in 45% of patients who have survived sudden cardiac arrest. Isoproterenol infusion is effective in suppressing VF episodes during electrical storms in BrS patients. Quinidine, a class IA drug, is effective in preventing recurrences of ventricular tachycardia (VT) and VF in a patient with BrS. However, quinidine is not available in most countries around the world, especially in Southeast Asia, where the disease is more prevalent. We present a case of BrS with an electrical storm while on therapy with oral cilostazol and discuss the management with oral quinine sulfate.

Combined Right Ventricular Outflow Tract Epicardial and Endocardial Late Potential Ablation for Treatment of Brugada Storm: A Case Report and Review of the Literature

A 34-year-old man with Brugada syndrome (BrS) presented with electrical storm, manifested as multiple appropriate shocks from his implantable cardioverter-defibrillator over a period of 7 hours. He had not tolerated prior treatment with quinidine, and had self-discontinued cilostazol citing persistent palpitations. After stabilization with intravenous isoproterenol, an electrophysiology study w...

Massive Electrical Storm at Disease Onset in a Patient with Brugada Syndrome

BACKGROUND Brugada syndrome (BrS) is a genetic arrhythmogenic disease characterized by ST-segment elevations in the right precordial leads of the electrocardiogram (ECG). These ECG changes may be concealed and BrS may present with electrical storm characterized by recurrent ventricular tachycardia and fibrillation. CASE REPORT A 49-year-old previously healthy man was admitted with electrical ...

Transient J-Wave Appearance in the Inferior-Lateral Leads during Electrical Storm in a Patient with Brugada Syndrome

A 67-year-old male patient was admitted with an abrupt sudden cardiac death. He represented with an extreme electrical storm of 30 times of ventricular fibrillation (VF) episodes on one day. External shocks were performed to terminate VF. Transient J-wave in the inferior-lateral leads and Brugada electrocardiography pattern on the right precordial leads appeared during the electrical storm. And...

Successful use of quinidine in treatment of electrical storm in Brugada syndrome.

We report an adolescent with a malignant form of Brugada syndrome who presented with 15 episodes of ventricular fibrillation (VF) over 10 days, shortly after implantation of an implantable cardioverter defibrillator. Oral quinidine bisulphate at a dose of 1000 mg/day successfully suppressed the electrical storm and recurrence of VF over 18-month follow-up. It also normalized the ST-segment elev...
